Anyone can refer a child/youth to be screened for CSoC services. If you are working with a child or youth who has significant behavioral health challenges and appears to be at-risk of out-of-home placement, call the member’s Healthy Louisiana Plan with the parent and ask for a screening for CSoC to determine eligibility. Magellan will coordinate with the Healthy Louisiana Plan to ensure that the member’s screening and eligibility are seamless.
How is Wraparound different from other community-based services?
Wraparound is not a service. Rather, it is an intensive, individualized, team-based planning process that aims to keep at-risk youth in their homes and communities. The Wraparound Facilitator and all team members work together to create and implement an individualized plan of care for the youth and their family.
